Patient Information

Your First Visit

 

We require that you fill out a Medical and Dental Health questionnaire.
In the course of your Dental treatment, you may be prescribed medication.
Some medications interact negatively with other medications being prescribed by another health care provider.
Some medications also interact negatively with some Medical conditions.
Your overall health is important to us, not just your Dental health.

Insurance Coverage

 

We require complete information of patient’s dental insurance coverage.
For your convenience, we will submit your claims electronically to your Insurance Provider.
Some Insurance Providers require mail in claims only. We will do that for you, on a standard dental claim form signed by the insured member.
Patients (or parents, guardians) are ultimately responsible for payment of all dental services provided.
We expect payment for all charges not covered by an Insurance Provider.

Appointments

 

We do our best to be on time for your appointment. We appreciate the same courtesy.
Occasionally emergencies do happen, which may delay our schedule. We apologize for any inconvenience such delays may cause you. Your understanding will always be appreciated.
If for any reason you are not able to keep your appointment, we would appreciate two business days’ notice.   Another patient in need may benefit from that scheduled time slot.

Registration Information

 

New Patients:

  • Please print and complete the patient information questionnaire.
  • Bring it with you, when you come for your appointment.
